FOR PARENTS/GUARDIANS [APPLICABLE FOR MINORS BELOW THE AGE OF 18]

• I confirm that -

  • I am the Parent or Legal Guardian of the Child (name reflected above), and that I have the authority to enter the Child into the Art Jamming Session.

  • I understand and will ensure that the Child will be accompanied and supervised by a responsible adult (either myself or any person above the age of 18 authorised by myself) at all times

  • I am fully aware of the risks associated with participating in the Activities at the Wildflower Studio. These risks include damage to or loss of personal property, personal injury.

  • I will comply with all instructions given to me by the Wildflower Studio staff, interns and volunteers during the Activities, and will ensure that the Child comply with such instructions. I confirm that the Wildflower Studio, its staff, interns and volunteers will not held liable if I or the Child fails to comply with any such instructions.

  • I acknowledge and accept that the Wildflower Studio shall not be liable for any loss or damage arising from the Child’s participation in the Activities, including indirect or consequential loss or damage.

Nothing in this disclaimer shall be construed as limiting or excluding Wildflower Studio’s liability for:

• a) death or personal injury which arises as a consequence of the Wildflower Studio’s negligence, or the negligence of the Wildflower Studio’s staff; or

• (b) fraudulent misrepresentation; or

• (c) any other matter for which it would be illegal or unlawful for the Wildflower Studio to exclude or attempt to exclude its liability.
